I am getting a little bit confused with this subject. On the edit tab and record flags options it shows all the census and three extra flags I have created birth, marriage and death should all these boxes be ticked.
I have created icons for all the census and they show up on my diagrams, this I think is down to Gedcom rather than me.
I have now created icons on the diagram boxes for birth death and marriage certicates but they only show up on a few people even though I have this information on the individual records.
Should I be working with a list and if so how?

I think you are missing the obvious. You need to set the flags!
I would use a query to find all the people with a birth certificate source on their birth event and use the result to set the flag or simply work through all your certificates ticking the people who you have certs for.
Flags are completely under your control FH does nothing automatically to set them.
Gedcom census sets census flags if you want when you enter census information.
