How do I add a "Relationship to Parents" column to a query (i.e. b= birth, a= adopted etc)
I can't find the field in the query "Fields" list, and I haven't been able to find the correct field name in the knowledge Base to be able to add it manually.

Try
Configuring the Record window, I found it by selecting <Other...> when configuring columns and then Insert Data Ref and browsing the reference tree.
In a query, you're looking for (Relationship) under Family as Child.

If an individual has more than one set of parents, you'll need more than one column, e.g.
