From my Individual Report:
"Roy Russell McDonald, son of Raymond Russell McDonald and Grace Marie Thompson, was born on 26 October in Renton, Allegheny County, Pennsylvania. [From my birth fact, dated first]
Roy Russell McDonald (known as Russell) was enumerated....." [From the Census(family) fact, dated second]
The Individual Reports Help says if you check Inc. Alternate names then if a person had multiple names, include a list of alternate names in brackets, prefixed by "also known as", after the first use of their name in their section. To me, the birth fact is the first use of their name in their section. Unless I don't understand what "section" means.
Given the settings shown below I desire (and expected) it to say:
"Roy Russell McDonald (known as Russell), son of Raymond Russell McDonald and Grace Marie Thompson, was born on 26 October in Renton, Allegheny County, Pennsylvania. [With the alternate names in the birth fact]
Roy Russell McDonald was enumerated....."
I also thought there would some reference to the nickname.
I can likely modify my birth fact to do what I want, but wondered what I misunderstood.

Jackie, your screenshot shows that you have no alternate names defined for this person. The Nickname and Given Name Used are not 'alternate' names as defined by FH. Only names entered in the lower section of the Names and Titles dialog will be included.

Jackie, there are some unusual details being shown and some misunderstandings in your comments.
Firstly, as Lorna says, the Inc. Alternate Names option only applies to Alternate Names in the lower pane of the Names & Titles dialogue and Roy Russell McDonald has none. So that option has no effect.
It is the Given Name Used that is reported using '(known as Russell)'.
The odd thing in your Report is that the first occurrence of 'Roy Russell McDonald' is not bold.
It is the Census(family) sentence that shows his name as Roy Russell McDonald in bold. I think that means FH believes that is the first occurrence of his name and so that acquires the '(known as Russell)'.
However, it is a Census(family) fact so it should show the names of the couple, not just the husband. Very odd.
Nicknames have to be included using an appropriate Name Qualifier or other specific Sentence Template code.
It would help to see a screenshot of the Narrative Report, the Individual Facts tab, and associated Sentence Templates.
