Family Historian User Group

Is it possible to modify this questionnaire sheet? It appears to be in 'read only format' only. Suggestions please.

If this refers to the Family Group Sheet which can be downloaded from Family Tree Magazine at http://www.familytreemagazine.com/info/basicforms there is a choice of:
PDF which will be read-only but can be printed using a suitable PDF reader (e.g. Foxit), or:
TEXT which is a Word document.
You should be able to modify the Word document but some browsers sometimes make all downloaded files read-only (not a problem with Google Chrome!).  If this has happened you should be able to modify the file properties using Windows Explorer.

Just use the Report options to customise it or save the output to an RTF file so you can edit it in word.
