Can I alter the Family Group Sheets or Individual Summary Reports to show attributes which are missing ie. a Questionaire type sheet which can be sent to relatives?

Save the report you want to use as a custom report then simply add the events you want to prompt for to Show rather than Hide in the Main section, adding extra events as you need them.
Its also worth making a exclude list for the Events section to exclude any events from the main section.
