Individual/family record forms

Can anyone point me at any formats for recording information for individuals and families. I feel I would like some non-computerised records for individuals showing: i)  Variations on names throughout a life
ii) Baptism and birth information
iii)Census locations and occupations at that time
iv) Marriage date(s) and spouse(s)and location(s)
v)  Childrens names and DOB's
vi) Death and burial dates and locations
I'm thinking of a family sheet too which could show the family members for each individual with names and birth/death dates.

Am I going retro? Is this a good or bad idea? I can't imagine I'm the only person to want something manual but haven't found a form(s) to do this so far. Any links,suggestions, or examples most welcome.

In the Knowledge Base FHUG Downloads under Family History Research there are several Report and Spreadsheet blank forms for recording such details:

I also like to have paper records, but use the various FH Reports customised to my needs to print the details in a consistent and tidy format.
