I have several such certificates and since about 1970 give the following:

4. Maiden surname of woman who has married:

6. Occupation and usual address:
7 (a) Name and surname of informant:
7 (b) Qualification:
7 (c) Usual address:
8. Cause of death:
9. Certifying signature of Informant
10. Date of registration:
11. Signature of Registrar:

Date and place of birth replaced Age at Death in Q2 (June) 1969.
Age at Death was introduced in Q1 (Mar) 1866.

7 (a) Name and surname of informant
7 (b) Qualification
7 (c) Usual address

These or something very similar have been present at least since about 1900.
