Hello
One of my grandchildren has changed his name at a very early age (at least his mother did it for him).
Should his original name be shown on the tree with an AKA reference , or should the Change of name be shown?

You might find how_to:handle_people_with_multiple_names|How to Handle People With Multiple Names, as a rule of thumb I would record all name changes with a name change event as it makes it easier for people in the future to understand the changes.
