Those Report Options look like the Installation Settings (except for the Pictures tab) so nothing unusual there.
I have tried several experiments using a small Project but otherwise with similar settings to yours for Multimedia, Reports, and Website Wizard but still cannot get FH to misbehave.
The only oddity I have found is that it is possible to temporarily have different Report Options settings for each ToC Report Item even though they all use the the same Report Type, and create a different style Report for each Report Item. However, once the Website Wizard is closed the temporary differences are resolved, and next time the Wizard is run they are all the same again. So can you confirm the following settings apply:-
Do you use the same Individual Summary Report (for Web, CD or DVD) for Step 3 - Choose Report Template > Primary Page Type and all five Step 4 - Add Items for Table of Contents?
Also confirm that the same Report Options apply to all six cases, especially the Pictures tab settings?
Have you rebuilt the entire website with the above settings, i.e. Max Pics all 0 except for Individual = 99?
Or do you use any shortcuts to avoid rebuilding everything, because it takes too long for your 20,000 Individuals?
e.g. I could imagine selecting just a few Individuals at Step 2, build the ToC Reports quickly, and upload them separately.

I have stumbled upon a possible explanation for the problem, but it needs a particular sequence of events as follows for the Individual Summary Report (for Web, CD or DVD).
1) The Report Options must initially have Pictures tab Max Pics all set to say 99 and a Report created for a significant selection of Individuals so that Spouse & Children images are included. These options must also be established as the settings for the Create a Website wizard Step 3 and Step 4.
2) Use Publish > Create a Website and at Step 3 - Choose Report Template set Options so Pictures tab Max Pics all set to 0 except for Individual and click OK.
3) Quickly click Next > several times and eventually Finish to create the website pages.
4) In the Website Created window click View Website and then Return to Website Creation Wizard. The website pages for Individuals will correctly omit Spouse & Children images, but the ToC Reports will often include those images as per the problem. In the wizard, click < Back to Step 4 - Add Items for Table of Contents and the Report Options will often still have Pictures tab Max Pics all still set to say 99!
5) However, if instead of Return to Website Creation Wizard the more usual Close Wizard option is chosen, then on revisiting Publish > Create a Website the Step 4 settings for Max Pics will correctly show 0 giving the impression that those settings were used to create the ToC Reports.
Unfortunately, the above sequence is not guaranteed to demonstrate the problem, as sometimes FH creates the ToC Reports correctly.
The solution is to ensure all the Step 3 and Step 4 settings for Report Options are correct before creating the web pages.
I plan to look into the symptoms more closely and report my findings to Calico Pie on Monday.

Thank you for the offer of your Carbonite backup, but I think I shall decline.
Perhaps Calico Pie will take up that offer if they cannot resolve your problem.
It might be worth clarifying your PC and FH configuration, just in case there is anything unusual about it.
I believe you are running a Windows 8 PC and presumably a standard FH installation configuration.
i.e. FH uses the default folders for its customisation files, the My Documents\Family Historian Projects folder for Project files, etc.
Please let us know if there are any non-standard features about your PC or FH.
What may be worthwhile is to download and install the latest FH V5.0.9 from http://www.family-historian.co.uk/downl ... ee-upgrade.
This has had some updates specifically for Windows 8 but without changing the Version number because they only affect the installation process, but it will do no harm to try it.

Select the Individuals required using your Query, and check the Report Options are as desired.
Use Save Report As > Web Page (HTML) and change filename to toc1 before clicking OK.
In the \Public\FH Web Page folder the toc1.html file will need the <div id="menubar"> section inserting at the start of the <body> and your script at the end of the <body> section from any HTML file in the \Public\FHWebsite folder.
This must be repeated for all Reports toc2, toc3, toc4 & toc5.
These files can then be copied to the \Public\FHWebsite overwriting the matching five files there.

Your alternative Projects folder is not a problem, and everything else is standard, so no problems there.
Please try the latest V5.0.9 update, because as I said it has changed simply to cope with Windows 8 installation issues, but will not be offered via the FH Check for Updates. You have to go to the FH website to download it. It may or may not fix anything, but just re-installing any software has often been known to fix problems.
I cannot remember checking, but if you use Publish > Miscellaneous Reports > Individual Summary Report (for Web, CD or DVD) and set your desired Report Options do you get the desired output on screen in FH?
After the FH re-install, the first step is to simply rerun your Create a Website wizard to see if the ToC Reports are fixed.
If not, then workaround (1) is not a headache at all:

Okay, thanks, I get it now. Yes, so I'd generate the custom reports and save them, but not in the main folder. Then I'd generate the website as usual and then copy the reports over into the main folder after completion.

You don't need to recreate the website pages as they presumably still exist in your \Public\FHWebsite folder?

I was thinking more of a quick fix for the current configuration now.
But yes, your plan would be the approach for the future, if the fundamental problem cannot be fixed in the meantime.

I won't know until I build the site from scratch though will I? It takes a few hours, so don't really want to try to try it until the site is ready for rebuilding. When was the latest 5.0.9 released?

You will notice that filename is now upgrade_fh5_to_5.0.9b instead of upgrade_fh5_to_5.0.9 and I think it was changed around the middle of last year after several users had problems installing on Windows 8.

But simply the act of re-installing has been known to fix inexplicable problems that nobody else can reproduce.
