Perhaps you have accidentally created two Media Records for the same image. If so, one of the Media records might be unused.

It's possible to have two Media records with the same name but different record ID numbers. If you sourt the Media list alphabetically do you see any duplicate titles?

I don't know of a way to get the text in the column heading to wrap, but you can adjust the width of the columns. (That will of course obscure some of the text in the heading.)
