Sorry Nick, I though the original problem is that the table looks all nice and neat in AS, but when you open the source text in FH the columns are all word wrapping. That is what I am getting.

I create a source for the headstone itself and add an image (If I have one) with a transcription in the text from source. I the add a source citation to each fact gleaned from the transcription. I do not use the burial fact because, as others have said, the person may not be buried there.

I have noticed this on the occasional UK census (usually 1911 but not certain). The thumbnail is there, but it is not visible.

It is not a problem with the latest version, it has happened for a long time, I have not been able to tie down what the cause is to report it.

When you add a connected or unconnected individual, they are not immediately assigned a pool number. That is only assigned when the data is saved.
